A speech bulb obturator is a type of dental prosthesis that is used to help improve speech in individuals with velopharyngeal insufficiency, which is a condition often associated with a cleft palate. 

The appliance consists of an anterior part (similar to a retainer or denture base) and a posterior "bulb" that is custom-made to fill the gap between the soft palate and the pharyngeal walls during speech. 

This helps to achieve velopharyngeal closure, control nasal emission of air, and prevent nasal regurgitation of food and fluids during swallowing. 

The surrounding pharyngeal walls move to touch the bulb during speech, promoting functional competence.
